Converting a Karameikos Campaign to 5E

What would need to be converted to run an AC1000 campaign in Karameikos for instance?

The Grand Duchy of Karameikos (AC1000)
Source material: Gaz 1, AD&D Karameikos: Kingdom of Adventure, Adventures (B-series etc).

CLASSES
Common Classes: Bard, Cleric, Fighter, Paladin, Ranger, Rogue, Wizard
Uncommon Classes: Barbarian, Monk
Rare (Check with DM): Sorcerer (see magic below), Warlock (see magic below)

RACES
Common Races: Human (Thyatian), Human (Traladaran), Human (Mixed), Elf (Callarii), Elf (Vyalia), Halfling, Dwarf (Stonehollow), Dwarf (Hill), Gnome (Highforge), Gnome (Forest), Hutaakan, Centaur
Uncommon Races: Goblin*, Eladrin (Sidhe), Half-Orc*, Hobgoblin*, Gnoll*, Bugbear*
Rare (Check with DM): Tiefling, Dragonborn, Goliath, Aasimar,
*=These races are quite numerous in Karameikos, but are rarely part of adventuring parties and are generally not tolerated in cities, towns etc.
